Is TRT Right for Me? A Virginia Physician’s Perspective

I hear this question almost every day at our clinics: “Doc, is TRT right for me?” It’s a fair question. Testosterone replacement therapy gets a lot of attention online, and not all of it is accurate. Some sites make it sound like a miracle. Others make it sound dangerous. The truth, as usual, is somewhere in the middle.

I’m Dr. Marvin Oats, Medical Director at Men’s Wellness Centers in Virginia. I’ve supervised TRT for thousands of men across our three locations. Here’s my honest take on who should consider it, who shouldn’t, and how to decide.

What TRT Actually Is

Testosterone replacement therapy is exactly what it sounds like: replacing testosterone that your body isn’t producing enough of. It’s physician-supervised hormone treatment, not a supplement or a booster you buy at GNC.

Your body naturally produces less testosterone as you age. For some men, levels drop enough to cause real symptoms: persistent fatigue, weight gain (especially around the midsection), mood changes, reduced motivation, and decreased drive. When blood work confirms low testosterone, TRT can help restore those levels to a normal range.

Signs You Might Benefit from TRT

No responsible physician will put you on TRT based on symptoms alone. But these are the patterns I see in men whose blood work eventually confirms low T:

  • Constant tiredness that sleep doesn’t fix
  • Gaining weight without changing diet or exercise habits
  • Feeling irritable or flat for no clear reason
  • Decreased motivation at work and at home
  • Brain fog or difficulty concentrating
  • Reduced physical stamina

These symptoms overlap with other conditions like thyroid problems, sleep apnea, vitamin D deficiency, and depression. That’s exactly why blood work is the starting point, not the finish line.

Who Should NOT Start TRT

TRT isn’t for everyone. I’ll be straight about this because a lot of clinics aren’t:

  • If your testosterone is normal, TRT won’t make you “better than normal.” We don’t prescribe it for men with adequate levels
  • Men actively trying to conceive should discuss TRT carefully with their physician, as it can affect fertility
  • Certain medical conditions need to be evaluated before starting any hormone therapy

What to Expect If You Start TRT

Most patients notice improvements within 4-6 weeks. Energy tends to come back first. Mood stabilizes. Motivation returns. Body composition changes happen more gradually over months.

We monitor bloodwork regularly. I adjust protocols based on how patients respond, not on a fixed schedule. Some men need more. Some need less. The goal is finding what works for you specifically.
